2019, Trends in NeurosciencesCitation Excerpt :Anxiety has been found among the primary psychiatric manifestations in patients with mitochondrial disorders, along with mood disorders, cognitive impairments, and psychosis [63,64]. Specifically, increased levels of reported anxiety-related symptoms, such as phobias and panic attacks with agoraphobia, were found in patients with an adenine-to-guanine mutation on mitochondrial transfer RNA, which is associated with diabetes mellitus [65]. Interestingly, a notable predisposition to develop high anxiety levels was also reported in mothers of mildly affected children suffering from maternally inherited mitochondrial disorders compared with mothers of children suffering from other metabolic diseases [66].

Of 205 patients with diabetes mellitus, 9 (4.4%) were found to be positive for the m.3243A>G mutation.

Four met comorbid criteria for mitochondrial disease and major depressive disorder (MDD).22–25 There is one case of MDD with psychosis,26 one case of bipolar disorder,27 eight cases of psychosis,28–35 and one patient with personality change;36 three cases with mutation in mt tRNA had anxiety disorders.23

The high frequency of comorbid depression was reported in patients with mitochondrial diabetes mellitus caused by the 3243 mutation of mtDNA (Miyaoka et al., 1997), and there is a case report of depression caused by the 3243 mutation (Onishi et al., 1997). Kato et al. (2001) screened the 3243 mutation in leukocyte DNA in 135 patients with bipolar disorder using the restriction fragment length polymorphism (RFLP) PCR method, but none of the patients had the mutation.
